The NAACOS Spring 2016 Conference will deliver valuable policy insights and operational strategies for all accountable care organizations, whether they are new or more established. The two-day conference will draw more than 400 ACO leaders and will feature presentations from noted ACO experts and CMS officials, as well as opportunities for exhibiting, networking and peer-to-peer learning.

As part of the Exhibit Showcase, Synaptic will demonstrate how clients such as Reliance ACO are improving outcomes and generating savings utilizing Synapse. The Synaptic team will illustrate how Synapse is helping ACOs consume and organize Admit, Discharge, and Transfer (ADT) notifications from Health Information Exchanges (HIEs).  In addition, ACOs will see how to use real-time ADTs inside of Synapse to prevent unnecessary admissions by engaging an ACO physician when a beneficiary registers in the ED; manage care transitions or care coordination when a beneficiary is discharged; and provide care management to those in need.
